MEDICAL TRANSCRIPTION DOCUMENTATION PROCESS

Medical Transcription Theory

1. Patient visit a health care provider.
2. The provider thoroughly evaluates/ treats the patient
3. After the patient leave, the provider dictates the patients’ health care records in a microphone.
4. The above voice is saved into a system.
5. This saved file is then sent to a medical transcriptionist as per the experience level.

6. The medical transcriptionist transcribes/ converts the file into text format.
7. The file is then uploaded the hospital records for the patient.
8. The medical coder apply codes to the file as per relevance.
9. The medical biller bills the visit from the coded document.
10. The insurance and the patient are billed.
11. The completed documents are filed for the patient
